"Big n' Fat" costs 3, which is a perfect items that has 3 enhancement slots. those 3 slots are the Cost, in other words the requirement for that effect(over health) to be applied. As a solid example: The first fish is a rare one meaning it has only 2 enhancement slots, it cannot have the "Big n' Fat" enhancement as this requires 3 slots in order to be applied. On the other hand the second fish is a medium fish of perfect quality that has 3 enhancement slots, but you still you can't apply "Big n' Fat" enhancement on it as it's not a 'large' or 'monster' fish. Basically 1 "Big n' Fat" enhancement can be applied only on items (that fall under the 'large fish' or 'monster fish' categories) of perfect quality that have 3 enhancement slots. As a side note it takes only 1 material to 'craft', the cost not being in materials. I know I've probably repeated myself several times right there with that explication.
