While Sera's instability (emotionally and literally when holding the spear) was to blame, more of the calculations between hitting and not fell on the system. It was almost 'protecting' Mama Boar as if just to screw over Sera and discourage her. Chase knew that Kayaba wasn't that unfair, but it was getting hard to believe.
