      Calrex's shop for it became very popular to use and since then Ssendom made them too. He gave me the .ai file so I could edit mine with ease, much better than a template I used to use with the painful to edit pdf's.

    Just for reference in the Boss Raid, Takao hasn't handed out the amulets yet RP-wise. You can keep the stats edited in for the sake of convenience though.
